Compare the active that creates a helper, the item Abyss consumes, and the upgrade that strengthens a familiar. Each example explains the trigger and the tradeoff.
The active item determines what a wisp does. Check whether it fires with you, waits for an activation, or needs invulnerability.
These are pedestal conversions. Decide between taking the item and consuming it for a locust, using the rules for your game version.
A familiar's attack can grow stronger without changing Isaac's HUD damage. Its firing position and attack timing still matter.
Blue flies have their own damage upgrade. Separate the helper that produces them from the effect that makes them hit harder.
