How to fight Duke of Flies
The Duke of Flies surrounds himself with flies that block tears, then sends threats toward Isaac.
Normal boss behavior in Repentance. Champion variants can change these patterns.
Before entering
Leave space to clear pursuing flies. Aim through an opening in the orbiting group when it is safe.

Orbiting shield
Watch for: Flies circling the Duke can absorb your shots while he drifts around the room.
Try: Keep firing to thin the group and adjust your angle without walking into the boss.

Released flies
Watch for: Larger flies chase you, and the orbiting group can break away toward Isaac.
Try: Clear nearby pursuers before they cut off your next sidestep.

After the boss falls
Watch for: Remaining orbiting flies are released, and Repentance adds flies on the Duke's death.
Try: Keep dodging and shooting until the room is clear before approaching the reward.
