Blood Donation Machine
Exchange current health for money, with a chance of a Blood Bag or IV Bag payout.
Payment and play rules
- Red health is spent first. Paying without enough health can kill you.
- A normal payment drops one to three coins; Hard mode reduces that to one. Keeper variants can receive zero or one.
- Only one IV Bag can spawn per run, while multiple Blood Bags can appear.
Before you spend
Use nearby spare Red Hearts to support payments, and stop before your next payment would be lethal.
Possible payouts
These are possible results, not a promise that every payment gives a reward.
- Coin pickups
- Rarely Child's Heart
Final payout or milestone
The machine breaks and awards Blood Bag or IV Bag when it gives its final item.
What happens if you bomb it?
Bombing it ends its use and drops coins or hearts. This does not guarantee a final collectible.
Recognize the machine

Original game frames. Counters are part of the artwork preview. The broken frame does not mean every machine can be bombed; read the destruction rule below.
Check your character before paying
The standard heart cost is not a safe-payment guarantee. Characters with unusual health systems need their own health rules checked first.
